Inclusion Healthcare are piloting a reimagining of Homeless Healthcare in Leicester and are looking for a skilled and experienced practitioner to help us. The Inclusion / UHL Homeless Healthcare pilot is about bringing together primary & secondary health & social care services to improve how people experiencing homelessness access & engage in healthcare.

- You’ll be working innovatively across primary & secondary care healthcare services & organisations.
- You’ll be joining a small multi-disciplinary team seeking to improve the health & wellbeing of individuals experiencing homelessness in Leicester City.
- You’ll be part of team that embraces and embeds lived experience throughout the health system.
- You’ll be part of a systemwide multi‑agency team addressing some of the most complex barriers to effective care.
- You’ll help to shape a model that could become the blueprint for future inclusion health work.
- You’ll see the impact of your work – in primary care, the Emergency Department, hospital wards, outpatient clinics, hostels, day centres and in the community but most importantly you will see improvement in the health of our patients.
Who we’re looking for Someone who is:
- Experienced in homelessness, substance use, Inclusion health or general healthcare either within primary or secondary care
- Compassionate, trauma informed, resilient and unafraid of complexity.
- Skilled at building partnership relationships across service & organisational boundaries.
- Motivated by reducing health inequalities.
- Excited by innovation and system‑wide collaboration.
- Confident working across multiple agencies and environments.
- Has the ability to build trust with people who may be hard to reach.
- Motivated to be part of a pilot that is genuinely innovative.
