The Changing Futures programme aims to improve outcomes for adults experiencing multiple disadvantage – including combinations of homelessness, substance misuse, mental ill health, domestic abuse and contact with the criminal justice system. The programme provides intensive support to individuals facing multiple disadvantage. This is achieved by supporting them to access other services (such as drug treatment, social care) through our specialist team members.

We also aim to identify barriers within the systems that prevent the progress of individuals facing multiple disadvantage, and to develop new pathways which enable people to make progress, at the point when they are ready to access a service (reachable moment), in order to overcome the barriers that have been identified.

We are open Monday – Friday (excluding Bank Holidays) : 08.30 – 17.00
